Investigations

In any progressive safety culture underlying causes (root causes) of accidents are identified through recognition and investigation of unsafe conditions and unsafe behaviors.

These services include the investigation of accidents, injuries and near misses in regard to workplace situations and public places.

Internal investigations for organizations are intended to identify why the incident occurred and what needs to occur to prevent similar situations occurring in the future. Such an analysis identifies the range of factors that contributed to the incident; the type of factors examined are:

  • Equipment factors – selection, use, maintenance, design, etc.
  • People/Behavioral factors – selection, training, experience, Leadership, etc.
  • Environmental factors – work environment, weather, lighting, etc.
  • Material factors – selection, handling, processing
  • Procedural factors – instructions, work practices, systems of work, etc.

Training can be provided to persons within a organization on how to objectively conduct an investigation of an incident to reduce the possibility of similar incidents reoccurring.

A well prepared investigation report will assist legal practitioners in either establishing or defending a negligence action.
